What is the HEADCHECK COVID-19 tool?

It’s important to make sure that everyone is following the organization’s and your region’s COVID-19 protocols. To help with this, we added a COVID-19 screening tool to HEADCHECK Health. This includes a COVID-19 screening questionnaire for athletes to fill out before each game and practice, as well as automatic notifications sent to the team’s medical staff so they can follow up with next steps if needed.

There are three ways to complete the questionnaire:

  • The team’s medical staff member can login to their HEADCHECK account and help the athletes complete the questionnaire
  • Athletes can do it themselves using their HEADCHECK Athlete account
  • For minors, a parent/legal guardian can complete the questionnaire for the athlete using their HEADCHECK Guardian account
